Reimbursement Lost Materials Form

RPL has updated the Reimbursement for Lost Materials Form.
 
The updated form can be found in the P&P manual appendix on the N drive.
Old reimbursement forms should be recycled and the newest form should be printed and/or added to circ department master copy folders.
Hopefully these changes will help us to better include all info that is needed for the financial and tech departments when issuing reimbursements and tracking returned items.

Found Damaged
If an item is found on the shelf damaged Circ Staff should:
-Assess whether the item is able to be circulated
-If yes, use the damaged stamp/date/initials
-If no, change the status to “Selector Assessment” in TLC
-Sticky note the item: “damaged, reorder/weed?”
-Send to the Selector
 
Returned Damaged
If an item is returned damaged Circ Staff should:  (wet/moldy/heavily stained/etc)
-Assess whether the item is able to be circulated
-If yes, do not charge the patron - use the damaged stamp/date/initials
-If no, charge the patron for the amount as outlined in the manual
-When the patron pays for the item, it becomes their property
-If the patron does not want the item, Circ Staff should mark the item “Weeded Item” in TLC
-Remove the security and dispose of the item
-Email the item info to the Selector
 
Item Not on File
If an item is returned and the screen shows “No Titles Found” upon check in Circ Staff should:
-Fill out a tech slip “Item Not on File”
-Send the item to the appropriate Selector
 
-The selector will determine if they want to add the item back into the system or weed it from the collection

*If Circ Desk Staff are not comfortable making the assessment on whether or not an item is able to be circulated, they should ask a supervisor

West Branch Donations

Circulation Staff:
 
If a patron designates a donation as for the West Branch Project, please fill out the form attached and add the charges as seen below:
 
 
If you receive a donation for the West Branch, please fill out a West Branch Donation Form (N:West Branch Project/West Branch Donation Form) and enter the transaction under Donation-West Branch in the LS2 system.  Turn the form in with your money and send to Administration.

Z_ROC Aged Out Patron Types


Z_ROC Aged Out Patron Types


This patron type is set to automatically update each day, based upon the former student’s DOB.

Library staff should not set this patron type for any cards.

If a Z_ROC patron attempts to use the computer or check out any items, their card will not work.

If you encounter a Z_ROC patron, circ staff should:
-Search by DOB to see if the patron already has a juvenile or adult card on file
-If yes, then circ staff should update the borrower info on the current card
-If no, then circ staff should treat the patron as if it is a new card and ask for ID, fill out application, initial the policies, etc.
-After completion of the new physical card, circ staff may delete the Z-ROC card from the system

Unattended Child Policy

Know the Unattended Child Policy.
                    Children 8 years of age or younger are to be “attended” by a parent or other responsible caretaker (16 years or older) during their entire visit to the library.
                    A child 8 years or younger who is attending library sponsored programs need not to be accompanied by a parent or caretaker during the program. However, the parent or caretaker is expected to remain somewhere in the library for the duration of the program and must be present at the doors of the program when it is over.                          
                    Children 9-11 years of age must have a parent or caretaker (16 years or older) somewhere in the library.
  •     Only 11-17 year olds are allowed to hang out in the TEEN AREA. This is in effect during all operational hours. Noise volume is okay to rise in this area, but teens must follow library policies. Inappropriate behavior such as cursing or physical contact are not allowed. Teens must be respectful to staff and other patrons. Adult tutors cannot tutor teens in this area. If patrons complain about the noise level please explain that this area is provided for teens to gather and sometimes noise levels will rise. Assist the patron in finding a more suitable area to work.

Claims Returned

A.  Claims Returned
 
1.   If a borrower believes that an item as been returned:
-         Check the shelves for missing item, also look in places where it might be misplaced (ex. Search for Adult Fiction in YA, LP or PB).
-         Have a coworker check behind you.
-         Suggest the borrower look at home again.
-         Renew the item if it can be renewed.
-         If the item cannot be renewed, refer the situation to a full-time staff member or leave borrower’s information for a site supervisor to handle.

2.   A full-time employee or site supervisor should:
-         Select Borrower Info and click on the Extra Information tab.
-         Verify the Excused Losses is less than 2 (Excused Losses = Claims Returned).  Borrowers are allowed no more than two Claims Returned

-         If Excused Losses is less than 2, return to the Check Out screen and select only the item in question.  Select Claims Returned.
